Our Team

Medical

Local pulmonary and wound care physicians visit on a frequent, scheduled basis to formulate recovery plans, assess patient progress and provide expert guidance.

Nursing

A dedicated team of RNs, LVNs and CNAs is available 24/7 to provide attentive care and support to residents. The nursing team interacts with residents daily to ensure the optimal level of care.

Therapy

Experienced licensed therapists restore functional independence via carefully designed programs tailored to each resident and their specific recovery goals.

Diet & Nutrition

Licensed dieticians collaborate with Healwise physicians, chefs and residents to develop individualized dietary programs that maximize nutrition without sacrificing taste.

Facility Management

Healwise Care Center is cleaned daily by the facility management team. We take cleanliness very seriously and implement a rigorous deep cleaning and disinfecting routine in every corner.

Social Services

We only succeed when residents succeed. Upon entering Healwise, we collaboratively devise a custom, detailed post-discharge plan with resources, follow-ups, contacts and more.
